BOOKS, DVDs, and CDs:


We apologize but we do not accept returns on books, DVDs, or CD’s for refund so they are for final sale ONLY. However, if you have not used the books, DVDs, or CDs then we will accept them back for store credit towards one of our Health Programs/Consultations with one of our doctors in the clinic or a Phone Education Session. Please note that you must email us first at (customerservice@exsalus.com) to let us know that you are interested in doing this type of credit and then email us again at (customerservice@exsalus.com) when the product(s) have actually been shipped (with and estimated delivery date). You will have to pay for the return shipping and handling back to our clinic. Once receipt of the product (in new condition) is confirmed you will be alerted by email. Only at that point will you be allowed to set up your Health Program/Consultation or Phone Education Session with one of our doctors using that credit towards the total cost.



SERVICES:


Please note: We spend a lot of time and expense in advance of your actual scheduled visit to prepare your file, review your medical history, develop your tailored health plan, and build your personalized transition materials. These are among the many reasons we require a nonrefundable deposit as well as a specific amount of time prior to scheduled appointments for cancellation. We apologize for any inconvenience this may cause and thank you for your consideration and understanding.


1.All deposits are owed at the time of initial appointment scheduling. There are absolutely NO refunds of initial deposits and they are due at the time of first scheduling your first appointment. The doctors may allow you to wait to pay your nonrefundable deposit until the day of your initial appointment but that is up to the discretion of the doctor. If you cancel or reschedule that appointment the nonrefundable deposit is still due and will be charged. Nonrefundable deposit  is  $150 or 25% of the total cost of your program - whichever is higher.


2.A full refund of the time not used is available, but please note that the refund will be of any money remaining after subtracting the nonrefundable deposit as well as any office session/consulting time already used, billed at the recalculated, non-discounted rate ($550/session or $295/hr).


3.If you cancel an appointment more than 7 days before your next scheduled appointment, then you can reschedule without a fee. Please reschedule at your earliest convenience (payment plans will continue to be automatically billed to your credit card every 4 weeks, starting on initial date of service, until paid in full no matter when your appointment time is actually used).


    a.If you cancel between 72 hours and 7 days of your next scheduled appointment, then        

    you may reschedule for a $60 fee.

    b.If you cancel between 24 and 72 hours from your next scheduled appointment, then you

    may reschedule for a $95 change fee.

    c.You may not reschedule a block of time that is less than 24 hours away from its

    scheduled appointment. Please note that any cancellations or no-shows will lose that block    

    of time without any refund or ability to reschedule. We will continue any remaining time of

    your specific health package as scheduled and as per the rules above.


4.Unforeseen events may exclude people from some of the above rules and fees (please discuss these events with your physician directly as only he/she may reverse these rules/fees).


5.By signing below I acknowledge that I have read, understand, and am willing to abide by this rescheduling, cancellation, and refund policy.
